Implant placement

Breast implants may be placed either partially under the pectoralis major muscle (submuscular) or on top of the muscle and directly beneath the breasts (subglandular). This is commonly referred to by patients and online websites as 'unders' and 'overs'. The method for inserting and positioning implants depends on the type of implant, degree of enlargement desired and your body type.

Submuscular placement (under the muscle) is commonly performed on patients with little breast tissue and/or breast droop. A possible advantage of submuscular placement is that you may be less likely to "feel" the implants through the breast. This placement may make your recovery slightly longer and may be more painful.

Subglandular placement (over the muscle) is commonly performed on patients with existing breast tissue and/or breast droop. A possible disadvantage of subglandular placement is that you may "feel" the implants through your breast or see rippling, but this can be minimized with the implant choice. This placement may make your recovery shorter and less painful.

There are many variations of technique which allow breast augmentation to be performed in different ways. Each patient has different desires and her own unique breast shape. The formula that gives one patient an excellent result may be totally wrong for someone else. When I consult with a patient, I suggest the surgical procedure which I feel will work best for her as an individual - my suggestion may be quite different than the plan which worked well for her friend. I explain that everyone is different and needs her own customized surgical plan.
